PHF Medical Support

Medical Administrative Management
A Sgt Medical Administrator is established on the strength of RAF PHF (SNCO Med Support) to conduct the medical administrative management and coordination of non-effective personnel posted to RAF PHF. SNCO Med Support is responsible for the following actions immediately an individual is posted onto the strength of RAF PHF:
  • Ascertain who is responsible for the clinical care of the patient from OC RAF PHF.
  • Confirm directly with the patient that they are aware of the provider of their PHC and explain the role of the SNCO Med Support. Set up regular liaison with the patient.
  • Inform the patient that it is in their own interest to keep both their PHC provider and SNCO Med Support abreast of changes to their medical situation (for example, changes in treatment, admissions, discharges etc).
SNCO Med Support is to provide the following services to the relevant groups of patients:

a. Patients whose PHC is Provided by a Service Medical Centre:
  • Liaising with the parenting medical centre to ascertain the patient's condition at least fortnightly.
  • Update RAF PHF administrative staff at least monthly on the patient's medical progress/situation within the confines of medical confidentiality. Significant changes in the patient's situation are to be notified to OC RAF PHF immediately.
  • Ensuring that medical boards are arranged promptly by the medical centre providing PHC, in accordance with AP1269A, Lflt 2-03, Annex E. Advice may be sought from the nearest Regional Occupational Medicine Department (ROMD) where required (see Lflt 1-04).
  • Update relevant RAF PHF personal file with information authorised by the patient for disclosure within RAF PHF.
b. Patients whose PHC is Provided by a local civilian GP:
  • Hold and maintain the individual's F Med 4.
  • Ensure that, wherever possible, copies of all medical records are included in the F Med 4.
  • Monitor RAF PHF personal files and, if not current, contact the patient (or their Next of Kin) to ascertain their condition fortnightly.
  • Ensure that the patient has, where necessary, access to local care facilities for example, district nurse and palliative care services.
  • Update RAF PHF administrative staff at least monthly on the patient's progress/situation within the confines of medical confidentiality. Significant changes in the patient's situation are to be notified to OC RAF PHF immediately.
  • Arrange medical boards promptly in accordance with AP1269A, Lflt 2-03, Annex E, liaising with the appropriate ROMD as necessary.
  • Inform SMO RAF Innsworth immediately by fax, email or telephone, of any medical administrative or clinical difficulties encountered.
Clinical Management
Responsibilities of SMO RAF Innsworth. SMO Innsworth is responsible for liaising with the local GP of patients who are being treated as temporary residents, should the SNCO Med Support encounter difficulties and request assistance.

Responsibilities of the Designated Service Medical Centre. Where a Service medical centre is designated as the PHC provider the SMO is responsible for the clinical care and management of the individual as detailed below:
  • Registering and treating the patient providing that they are within a reasonable distance.
  • Holding the patient's F Med 4 and maintaining a PHCIS record in the normal manner.
  • Ensuring that copies of all medical records are included in the F Med 4.
  • Conducting or arranging medical boards in accordance with AP1269A, Lflt 2-03, Annex E.
  • Informing CMO(Health Plans)(RAF) immediately by fax (copied to SNCO Med Support RAF PHF), of any clinical difficulties encountered.
